DESCRIPTION: TEAMCARE is an EU Project funded by the European Commission that brings together partners representing education and training providers, healthcare authorities, employment and educational research, public authorities, and European networks to develop a new EU Curriculum for health and social care professionals delivering person-centered care in the community.\nThis Final Conference represents a unique opportunity to discover the TEAMCARE training package, connect directly with project partners and pilot teams, and explore how these resources can be effectively adopted and tailored to diverse educational, health, and social care settings across Europe. \nTEAMCARE has successfully developed and tested a comprehensive European training package designed to empower health and social care professionals working within Community-Based Interprofessional Teams.\nThe conference will guide you through the complete TEAMCARE journey:\n\nThe Context: Understanding the key challenges currently facing European health and social care systems.\nThe Solution: A detailed look at the TEAMCARE training package as a practical, scalable response.\nThe Field: Key lessons learned and insights gained from our four pilot experiences in Italy, Ireland, Greece, and Poland.\nThe Future: Interactive afternoon sessions dedicated to context adaptation, future adoption, scale-up, and long-term sustainability through practical guidance, Twinning Partnerships, and the Memorandum of Understanding.\n\n\nYour participation will also grant you access to the vibrant TEAMCARE Community, a growing network bringing together organisations and professionals dedicated to interprofessional, community-based, and person-centred care.\n
