On 2 March 2022, NWE-Chance presented its conclusions in a special joint event, organised together with Passion HF, and hosted by AgeingFit 2022. The project team was present in Lille, France for its final conference and held several interesting presentations on its lessons learned and how it plans to promote hospitalisation@home further.
NWE-Chance has investigated how different digital health devices can be integrated in a single platform. After the necessary development, the project has tested the platform in a feasibility study where patients who are recovering from heart failure have been admitted at home. Needless to say home hospitalisation is becoming increasingly important since, in COVID times, being hospitalised safely at home can often be more attractive than being admitted to hospital.
The presentations focused on the challenges faced to care for patients at home, the platform's openness to third party, its successful feasibility study and the Digital Innovation Hub: the platform that aims to reach out to all stakeholders (hospitals, technology partners, government, insurers, patients) and share expertise and lessons learned, making new partnerships, integrating and extending the technology platform.
